Mission Statement


To provide vocational and daily living skills assessments to the physically, emotionally, and mentally disabled person in the order to evaluate their needs and concerns for a maximum appropriate setting.



Admission Criteria


*16 years or older and age appropriate as determined by the individuals IDT.
*Must be a resident of counties served.
*Identifiable disability based upon medical, psychological and/or psychiatric evaluation.
*Must have a funded slot available in program.

*Intake *Case Management *Vocational Evaluation
*Transportation *Referral for medical,
dental and financial assistance
*Referral for ancillary services




Individuals Served

*Developmentally Disabled (IL-DHS-ODD) *Psychiatric disability
*Physically and/or medically diagnosed handicap *Learning Disabled
*Communication handicap *Socially, culturally and economically disadvantaged




Outcomes Expected

It is expected that the Vocational Evaluation program will provide realistic assessments that are consistent with the individual’s vocational interests and abilities and provide the individual with direction and services based upon the assessments and the individuals interests.
